Football has physical risks — even the non-contact skills work we teach. By signing, you acknowledge those risks, give us permission to seek emergency medical care for your child if we cannot reach you, and agree that you will not sue Wide Receiver 81 for the ordinary risks of sports participation. - Acknowledgment and assumption of risk. Football skills training — even non-contact drills, agility work, and conditioning — involves inherent risks including falls, collisions, sprains, fractures, concussions, heat-related illness, and other physical injury. The signing parent assumes these risks knowingly and voluntarily on behalf of themselves and the participant.
- Medical authorization for emergency care. If the participant is injured and the parent / emergency contact cannot be reached, coaches may administer or seek basic first aid, call 911, and authorize a hospital's emergency physician to provide care necessary to preserve life or prevent serious harm. The parent is financially responsible for medical care provided.
- Disclosure of medical information. The parent warrants that allergies, medications, conditions, and physical limitations have been disclosed in the registration form, and agrees to update Wide Receiver 81 if anything changes.
- Code of conduct. Parents and participants follow the program code: respect coaches and other families; arrive and depart on time; no aggression, bullying, or discriminatory behavior. Coaches may suspend or remove a participant for repeated or serious violations. No refund is owed for removal due to conduct violations.
- Release and covenant not to sue. The parent releases Wide Receiver 81, its founder, coaches, employees, contractors, and volunteers from claims arising from the participant's participation, EXCEPT claims arising from gross negligence, willful or reckless conduct, or intentional misconduct. Massachusetts law preserves those rights regardless of any waiver.
- Indemnification. To the extent Massachusetts permits, the parent indemnifies Wide Receiver 81 from third-party claims arising from the participant's actions (e.g., if the participant injures another participant). This does not extend to the released parties' own gross negligence, recklessness, or willful misconduct.
- Photo / media. Photo and video use is governed by a separate Photo and Media Release. This waiver does not authorize photo or video use; that permission is captured by a separate, opt-in checkbox.
- Severability. If any provision is unenforceable under Massachusetts law, the rest of this waiver remains in full force.
- Governing law and venue. Massachusetts law governs this agreement. Disputes are heard in Massachusetts state or federal courts.
This waiver is acknowledged via checkbox during registration. The checkbox click, the registration timestamp, and the parent's name and email are stored as the executed record.
